Name and Email fields used as primary contact in Contact Form program. You may add more name and email contacts below, up to twenty.

• Private use - means Department field below will use contact name field.
• Company use - means you can add more email addresses and names along with department names.

This field tells our program where "sendmail" program located on your server. Ask your system administrator if you don't know the answer.
If program location is invalid - you will not be able to use Contact Form program!

If "Yes" selected - Contact form will require First and Last names to be entered in a form.
If "No" selected - Contact form will just verify that name is entered.

Verify sender email

If "Yes" selected - Contact form will make email format check as user_name@domain.xxx
If "No" selected - Contact form will not perform email check at all.

Numbers only accepted. If nothing entered or zero (0) - Contact form will not verify comments length at all.
If small number entered (we recommend no more than 20) than Contact form will check if required number of characters entered in a comments field.

Send "Thank you" email to the client

If "Yes" selected - Contact form send "Thank you" letter to the person who filled out the form. Program will check the validity of email address prior to sending.

The value of this field will be used as your Page Title, it will be inserted in email letters sent to clients and on "Thank you" page at the end of the form submission.

Email, Name(s) and Departments

Additional Names, Email addresses and Department names can be provided. Contact form will be using all provided information to build appropriate Contact form.
Email addresses will not be visible on your web page at any time.

Four additional record entries provided initially. If you need more entries - just check appropriate checkbox and enter number of additional fields to be built on the next page.

This message will be shown on "Thank you" page and included in e-mail to the client if "Yes" selected in Send "Thank you" email to the client field.

Your HTML template page and HTML template code

You can apply your existing page design to your contact form page. Just get the source of your existing page design and insert !CODEINSERT! string where you want all your messages to appear.
You can upload this html file or insert HTML code. Uploaded file will be used if both fields are used.
